You will need to bring your insurance card so we can make a copy of it for our records. Texas state law requires that you have a referral for physical therapy from your physician, nurse practioner, physician assistant, chiropractor, osteopath, or dentist. Please make sure you obtain one before your appointment.

Print and bring in your completed New Patient Intake Forms or fax them to our office. This step will save you time on your initial visit.

Click on the links below to access the New Patient Intake Form. After you click the link, a browser window will open. From there, fill out the form and click on the 'Submit by Email' or 'Print Form' Button on the form. The Body Chart will have to be printed  to fill out and either faxed or brought with you to your appointment.
